In order to have more time to our disposal during your initial visit, we would like that you fill out the new patient form at least four/five days before your visit so we would have time to thoroughly review it.  Please click on ‘Book a Consultation’ to schedule your initial visit, and at that time we will get your e-mail address and send you on Microsoft Word format the ‘New Patient Form’, you can then fill it out and send it back to us as an attachment in all confidentiality.
